FGP: Foster Grandparent Program
Every teacher needs to fill out 3 Child Assignment Forms (CAF).
Child Assignment Forms tell a Foster Grandparent what child they are to work
with and what they are to do.
The CAF is filled out when the Teacher/Supervisor identifies a
child that needs the services of the Foster Grandparent.
If a child leaves your classroom, choose a new child for the Foster
Grandparent to work with and send a new CAF. If possible, please
complete (Evaluation) an CAF for the child who left.
